Minister Eric Hoskins’ Bill 41 continues to be confounding for many physicians, but possibly the most consistent question I am hearing is: Why do we need another layer of bureaucracy? How will sub-LHINs improve the system? One very intriguing twitter answer that I received suggested that this extra bureaucratic layer will serve as “administrative infrastructure” for primary care. That is something worth considering since I agree that Family Doctors need much more support than they are currently receiving.
Before I move directly into the discussion, I want to stress that I am not including those primary care providers who are not physicians in this consideration. The reason is that I want to focus on the infrastructure resources needed to deliver primary care and the Nurse Practitioner- led clinics are tremendously well-resourced, with all expenses already covered by the government, a luxury that physicians cannot access to the same degree which is the point here.

A new report by researchers at the Institute for Clinical Evaluative Sciences released yesterday provides an eye-opening look at how our healthcare dollars are being spent in this province.
This is an in-depth look at our healthcare spending at the level of each individual in the province. Researchers tracked every hospital admission, physician visit, home care visit, lab test, and drug prescription for 14.9 million Ontarians between 2009 and 2011.
They accounted for $30 billion, or 75 cents out of every healthcare dollar spent in this province, and found that 43 per cent goes to hospital care, 27 per cent to doctors, 15 per cent to drugs and lab tests, and 15 per cent to long-term care.
As it turns out, “high-cost users” consume a disproportionate amount of these resources. Ten percent of the population accounted for 77 per cent of spending and 1 per cent accounted for 33 per cent of spending. Each person in that top 1 per cent consumed almost $45,000 a year, compared to a maximum of only $333 a year for each person in the lower 50 per cent cost bracket of the population.

TORONTO — A three year old boy from Guelph, now has a surgery date set in the U.S., thanks to an outpouring of community support.
Bentley Mitchell has cerebral palsy and currently needs a walker to get around. SDR surgery, offered in St. Louis Children’s Hospital, could help him walk independently.
Bentley was the guest of honour at Granite Homes annual golf tournament in Guelph, Ont.
The tournament donated nearly $35,000 raised by participants to Bentley.
His parents had been working for months to raise $125,000 for the surgery and related costs. They have now met their goal.
“It’s been amazing the support we’ve had. You can’t even put into words how touching it is,” said Chad Mitchell, Bentley’s father.

This is a great article on the real cost of EMR in Ontario!
How much does is cost to care for a baby, pregnant woman, diabetic, heart disease patient, or provide primary care among the myriad of other services offered by your family doctor? According to the Ontario Government, too much. According to you and me?
I am not about to rehash the many exemplary articles and blog posts that have so eloquently outlined the issues (see attached PDF). My focus is to ask you a single question: Are you getting value for what is spent on primary care health teams?

It looks like another #OntarioMD approved EMR vendor is no more. In a follow up to our article “Instability with #EMR in #Ontario“, yesterday OntarioMD updated their approved vendors to indicate the following.
As of February 20, 2015 the Jonoke Software Corporation Inc’s EMR offering JonokeMed v5.5 is no longer a funding eligible EMR.
No specifics were listed, however the site indicated some type of “breach”. If you are looking for more information please click on the link below or contact OntarioMD directly.
